Dennis and Pam

Paul

Paul took on the guardianship role for his younger brother, joining their other sibling in one of our Supported Independent Living (SIL) houses.

The transition was a significant shift from the hospital care Paul’s brother was accustomed to, but we worked hard to help him adapt.

We collaborated closely with Paul to help him manage the complexities of the NDIS, including introducing him to a new support coordinator from an external organisation and unwaveringly continuing support even during funding challenges.

With a welcoming environment, the right nutrition and a consistent, caring team, Paul’s brother’s wellbeing has significantly improved. Despite the challenging disease affecting him, his SIL house has become a true home.

Sarah

After a six-month hospital stay, Sarah sought a provider to help her navigate the challenges of mental health and rediscover her goals and ambitions.

Care & Connects facilitated a smooth transition from the hospital and, with the support of our dedicated psychosocial team, Sarah is gaining confidence and building skills through educational courses.

Despite the ups and downs of living with a mental health diagnosis, the collaborative care and customised planning provided by Care & Connects are helping Sarah have more better days and make progress towards her goals.

Andrej

Andrej needed support for interstate travel to reunite with his family, but faced multiple rejections in his search for dependable support. That’s when Care & Connects stepped in to help.

We provided a 12-day period of Short-Term Accommodation (STA) support along with an accessible vehicle, enabling Andrej to make the journey.

After the initial travel support, we continued to provide community participation services, focusing on enriching Andrej’s social interactions and community engagement.

With personalised care from a dedicated support worker and a comprehensive approach to building independence, Andrej affirms, “Care & Connects are brilliant – they’re just good at everything!”
