Our Support Coordination services are designed to help you build the skills necessary to understand and effectively utilise your NDIS plan. Your dedicated Support Coordinator will work closely with you to ensure that you have access to a diverse range of supports that enhance your capacity to maintain relationships, manage service delivery tasks, live more independently, and actively participate in your community.

Specialist Support Coordination

For individuals with more complex situations who require a higher level of support, we offer Specialist Support Coordination. Our experienced Specialist Support Coordinators will work diligently to help you manage challenges in your support environment and ensure the consistent delivery of services tailored to your unique needs. This advanced level of coordination is designed to provide you with the expert guidance and resources necessary to navigate even the most complex support landscapes.ms.

Getting started is easy

Step 1: You contact us or submit a referral

You can either reach out to us directly or have someone submit a referral on your behalf.

Step 2: We meet to discuss your goals

We meet with you to learn about you – your unique needs and challenges, your goals and passions, and how we can help you.

We may even recommend other, external supports and providers to give you all the support you need.

Step 3: We match you with your perfect support team

We partner you with a team of support workers who ‘click’ with you.

You will be spending a lot of time together in the months and years ahead, so it’s important these are people who over time feel like friends and family.

Frequently asked questions

Support coordination focuses on advising/guiding participants to self-direct care and build capacity. Plan management handles invoices, payments, budget tracking. We offer complementary strengths.

For those with more complex situations like challenging behaviours, acute disabilities or remote access issues, our specialist coordinators have extra capabilities to facilitate environments for effective support.

To avoid any conflict of interest, if we supply any of your disability care services already, we will refer you to one of our partner agencies that provides independent, objective coordination.

Key responsibilities include developing your NDIS plan, coordinating services, liaising with providers, monitoring progress, resolving problems, supporting through plan reviews, and more.
