Eva

Eva suffered an ischemic infarct in the left middle cerebral artery, affecting a large portion of her temporal lobe. As a result, she has reduced active movement and strength in her right arm and leg. Despite the severity of her stroke and limited to no movement on her right side, it has not deterred her from setting goals and striving for independence.

She was transferred from Lyell McEwin Hospital to Care and Connects at the end of 2024. Since then, Eva has been focused on improving her communication skills and establishing a daily exercise routine. She collaborates actively with our staff and other professionals to enhance her quality of life.

“Determination” is the best word to describe Eva—a woman committed to transforming her current situation into a chapter she can overcome.

Philippa

Philippa is a lovely woman who resides in one of our Supported Independent Living (SIL) houses.

Her transition to Care and Connects was smooth and well-supported. Every detail and condition were carefully assessed to select the right staff and the most suitable home to help her achieve her goals.

We have provided dedicated and unwavering care, always prioritising her choices in the pursuit of independence.

From making healthy meal choices to establishing daily routines and increasing social and community participation, Philippa has demonstrated strong self-commitment to improving her life—and we are proud to be part of her journey.

Sarah

After a six-month hospital stay, Sarah sought a provider to help her navigate the challenges of mental health and rediscover her goals and ambitions.

Care & Connects facilitated a smooth transition from the hospital and, with the support of our dedicated psychosocial team, Sarah is gaining confidence and building skills through educational courses.

Despite the ups and downs of living with a mental health diagnosis, the collaborative care and customised planning provided by Care & Connects are helping Sarah have more better days and make progress towards her goals.

Andrej

Andrej needed support for interstate travel to reunite with his family, but faced multiple rejections in his search for dependable support. That’s when Care & Connects stepped in to help.

We provided a 12-day period of Short-Term Accommodation (STA) support along with an accessible vehicle, enabling Andrej to make the journey.

After the initial travel support, we continued to provide community participation services, focusing on enriching Andrej’s social interactions and community engagement.

With personalised care from a dedicated support worker and a comprehensive approach to building independence, Andrej affirms, “Care & Connects are brilliant – they’re just good at everything!”
