Psychosocial Disability Supports

Specialist, recovery-oriented psychosocial support

Support requirements for people with a mental health diagnosis are different to those for people with a physical disability. That’s why we have specialised teams and training – aiming to be a provider of choice for NDIS participants with a psychosocial disability.

Traditional disability supports are not enough

Living with mental health conditions requires specialised supports with knowledgeable professionals.

At Care & Connects, we have assembled a team of mental health experts to support you on your journey to recovery.

Our approach

Specialist support teams

Our dedicated psychosocial team comprises Certificate IV-qualified mental health workers and experts in recovery-oriented practice. They receive ongoing supervision and training to provide quality and compassionate support.

Evidence-based, recovery-focused model

Utilising strengths-based approaches, we support you to grow your capacity and independence. All staff are trained in delivering recovery-oriented practice, and receive ongoing training and supervision to support their practice and knowledge of psychosocial disability while adapting to your changing needs.

Collaborative care

We work closely with your coordinators, treating professionals (including positive behaviour support practitioners and psychosocial recovery coaches) and other stakeholders (including community mental health services and forensic services).

Customised care planning

Your personal psychosocial care plan is specifically designed to meet your individual situation and goals and delivered at your pace. We continuously review the support provided to ensure it aligns with your individual goals and requirements.
