Receive query and conduct assessment

When we receive a query from stakeholders, our team carefully assesses the request and completes a thorough risk assessment to ensure we can safely meet your needs. We begin the process of planning your transition from hospital to your preferred living arrangement by developing a tailored discharge plan.

Step 2

Offer suitable programs and conduct home readiness evaluation

Based on the assessment, we offer appropriate vacancies and support programs that align with your specific needs, goals, and NDIS funding. If you’re moving into a Care & Connects house or your own home, we assess the living environment to ensure it is safe, accessible (with minor modifications if needed), and equipped for your return.

Step 3

Coordinate discharge and develop care plan

Once you’ve chosen your preferred program of support and accommodation, we arrange a discharge planning meeting to collaborate with hospitals and healthcare providers. During this meeting, we ensure all aspects of your transition are addressed and create a tailored care plan. We organise resources and prepare our service delivery team to provide you with immediate care support upon discharge, including medication management and assistance with daily living activities.

Step 4

Provide accessible transportation

On the day of discharge, we provide a free, accessible vehicle to transport you from the hospital to your home or Care & Connects house, ensuring a comfortable transition.

Step 5

Deliver ongoing support and NDIS compliance

After discharge, we provide ongoing support, with regular check-ins from our Registered Nurses. We handle all paperwork and coordination to align with NDIS guidelines, ensuring a faster discharge, usually within 1-3 weeks.
