Create a daily life that meets your needs
We understand the challenges that can make daily living difficult – barriers to mobility, lack of at-home support, and isolation from your community. Care & Connects’ disability supports help you overcome these obstacles so that you can continue living independently and freely.
Our community participation services get you out and about in the world to build your social connections and engagement with others.
And our in-home care means you can stay where you feel most comfortable rather than move into a facility.
This way, you can focus on living life to the fullest each day, entirely on your own terms.

From here, we utilise an innovative, evidence-based framework that focused on building capacity and developing your inherent strengths.
