Our approach
At Care & Connects, our community nursing services allow you to receive clinical care within the comfort of home. All our nurses are highly qualified and registered with the Nursing and Midwifery Board (Australia) with expertise in high-level health care, medication management, personal care, and psychosocial support. We believe in consistency, and we partner you with a dedicated team of nurses to ensure personalised and continuous care.

Complex bowel care
Our nurses provide comprehensive support for individuals with bowel dysfunction, including constipation, diarrhea, and fecal incontinence. They deliver personalised bowel management programs aimed at achieving regular and predictable bowel emptying, preventing accidents and complications, and ensuring optimal bowel health through physical and pharmacological interventions. Our services include stoma care and education on maintaining bowel health.
Enteral feeding and management
We offer comprehensive expertise in enteral feeding, managing nasogastric, gastrostomy, and jejunostomy tubes. Our services encompass formula selection and personalised feeding schedule planning to ensure proper nutrition.
Severe dysphagia management
For individuals with severe swallowing difficulties, our nurses provide advanced strategies for safe eating and drinking. This includes modified food and liquid preparation, positioning techniques, and oral hygiene maintenance.
Tracheostomy management
We provide specialised care for individuals with tracheostomies, involving the creation of an opening in the neck to insert a tube into the windpipe for breathing. Our services include tube changes, suctioning, stoma care, and education on proper tracheostomy management techniques to ensure airway patency and overall respiratory health.
Urinary catheter management
Our nurses are skilled in managing urinary catheters, used to assist individuals unable to empty their bladder. This includes insertion, removal, and care of different types—indwelling (urethral or suprapubic), external (condom), and short-term (intermittent) catheters. Our focus is on maintaining hygiene, preventing infections, and promoting overall bladder health for safe and effective urinary management.
Ventilator management
We provide support for individuals requiring ventilator assistance, including equipment setup, monitoring, troubleshooting, and education on respiratory health maintenance.
Subcutaneous injections
Our nurses administer subcutaneous injections with precision, ensuring proper technique, dosage accuracy, and appropriate site rotation to minimise discomfort and complications, such as those for insulin or heparin.
Complex wound management
For individuals with complex or chronic wounds, we offer specialised care including advanced dressings, debridement techniques, and strategies tailored to promote healing and prevent further skin deterioration.

You will be spending a lot of time together in the months and years ahead, so it’s important these are people who over time feel like friends and family.
Frequently asked questions
Where possible we will roster you with the same 1-2 nurses regularly to promote familiarity and continuity of care. However, we have a pool of experienced nurses who can fill in when needed.
We can visit daily or a few times a week based on your NDIS plan funding. Some only need occasional nursing visits. Let us know your ideal care frequency.
Yes, we have nurses available for overnight and 24 hour care. This includes help with tasks like continence management, medication administration and repositioning.
We liaise closely with your GP and other health professionals and document each nursing visit thoroughly. Let us know immediately if your condition changes between visits.
With your consent we collaborate closely with your other providers to coordinate care plans, share notes, and ensure continuity. Just let us know who is on your care team.
