Sarah's Story

After a six-month hospital stay, Sarah was looking for a provider who could help her build a life in the face of her daily struggles with mental health issues, while rediscovering her goals and ambitions.

Smooth transition from hospital

We facilitated Sarah’s hospital discharge, providing a free accessible vehicle on transition day and ensuring a smooth transition into her new living arrangements. We made necessary modifications to ensure accessibility and safety, and established a routine and daily schedule that includes time for self-care, rest, and enjoyable activities.

Specialist psychosocial support

At Care & Connects, Sarah found her pathway with the assistance of our dedicated psychosocial team, who utilise a Recovery-Oriented Practice approach. Our team, comprising Certificate IV-qualified mental health workers, receive ongoing supervision and training to provide quality and compassionate support.

Collaborative care and customised planning

We work closely with Sarah’s coordinators, treating professionals, and other stakeholders to ensure a collaborative approach to her care. Her personal psychosocial care plan is specifically designed to meet her individual situation and goals, and is delivered at her pace. We continuously review the support provided to ensure it aligns with Sarah’s evolving needs and requirements.

Building confidence and independence

Through the support of our team, Sarah has enrolled herself in a short educational course that she attends weekly, as well as a TAFE course. These opportunities are helping Sarah gain confidence in herself and build her interpersonal skills, with the goal of eventually obtaining employment within the community.

Navigating the ups and downs

Living with a mental health diagnosis means that Sarah experiences both good and bad days. However, with the Recovery-Oriented Practice approach and the network of supports around her, Sarah is having more better days and making progress towards her goals.

Sarah’s story shows that the right support to build independence can make a meaningful difference in the lives of individuals living with mental health challenges.
