Her remarkable progress is clear evidence of how unwavering care, combined with a positive attitude, can transform lives.

“I receive exceptional care and stay actively connected with my broader community.”

Anne's Life at Care and Connects

Anne enjoys a variety of outings and daily activities, from coffee trips to a nearby Café to scenic drives and shopping excursions. She stays closely connected with family and friends through regular phone calls, maintaining strong relationships that enrich her life.

Since moving into her new home, Anne has made impressive progress in adjusting to her routines. Supported by her Positive Behaviour Support (PBS) plan, she is actively working on managing behaviour, smoking habits, and bathtub safety. Her positive engagement and growing independence reflect her commitment to wellbeing and a fulfilling lifestyle.

Building confidence and independence

Anne has shown remarkable dedication to building confidence and independence in her daily life. With encouragement from staff, she actively makes her own choices during activities, strengthening her decision-making skills and enhancing social interactions. Her consistent participation reflects a strong commitment to improving her overall well-being and embracing opportunities for personal growth.

Determination in her mobility progress

One of Anne’s most significant achievements has been her progress in mobility. She now confidently uses her 4-wheel walker indoors with staff assistance—a milestone that demonstrates her determination to reach her independence goals. This accomplishment is not only a testament to her perseverance but also an inspiring example of how commitment and support can lead to meaningful change.
