Supported Independent Living (SIL)
A house that feels like home
Care & Connects’ Supported Independent Living includes a personalised care plan that aligns with your strengths and aspirations.
Housing options
Whether you choose to live in your own home, a leased property, or one of our group housing sites, we ensure that your journey towards independence is supported, comfortable, and tailored to your unique needs.
Consistent, caring support workers
Our support workers care deeply about their participants.
All support workers hold their Certificate III in Disability, NDIS
Clearance, WWCC, Police Check, and skilled in high-level health care, medication management, personal care, and psychosocial support.
We partner you with a team of support workers to ensure consistency of care.
Capacity building
We ensure that every aspect of our service is designed to help you achieve your NDIS goals in a way that feels right for you.
Specialist mental health SIL teams
For participants with a psychosocial disability, we provide Mental Health-qualified teams trained in recovery-oriented practice. Our focus is to maintain or build your independent living skills as much as possible to assist in your Recovery journey. See Psychosocial Disability Support for more.
Wheelchair accessible vehicles
We provide our own Wheelchair Accessible Vehicles, cutting out the coordination of a middle man. That way, your support worker can take you where you need, when you need.
Specialist Disability Accommodation (SDA)
Tailored living spaces that build your independence
Care & Connects’ Specialist Disability Accommodation is designed to build your independence. Each SDA home is set up for your specific needs.
Housing options
Our SDA options range from individual homes to shared living arrangements, each designed to provide a comfortable and accessible living experience. All accommodations are aligned with SDA Design Standards, ensuring improved liveability, robust construction, full accessibility, and high physical support.
Eligibility & funding
To access our SDA homes you will need to have SDA funding in your NDIS plan. SDA Eligibility is determined by the NDIS depending on your level of support requirement.
Medium-Term Accommodation (MTA)
Your stepping stone to independence
Medium Term Accommodation (MTA) offers up to 90 days of supportive living, ideal for transitions from hospital or rehab, or for exploring long-term living options like SDA or SIL.
Collaborative support
We collaborate with your support coordinator, healthcare professionals and broader support network to create a safe, homely environment.
Comfort and security
We focus on providing a secure, comfortable interim solution that caters to your needs and ensures you feel at home.
Respite and Short-Term Accommodation (STA)
A temporary retreat
Care & Connects’ respite and short-term accommodation (STA) services provide continuous, professional care while your caregiver has a break or you otherwise need temporary care.
Personalised support
We tailor respite care to suit the specific needs of each individual, from post-hospital care and recuperation to caregiver holidays.
Caring environment
We provide a secure and rejuvenating environment, helping both caregivers and participants to refresh and recharge.
Experienced caregivers
Our team of support workers all hold their Certificate III in Disability and are trained and skilled in various aspects of health care and support. Most importantly, they are highly caring and compassionate.

We meet with you to learn about you – your unique needs and challenges, your goals and passions, and how we can help you.
We may even recommend other, external supports and providers to give you all the support you need.
Step 3: We match you with your perfect support team
We partner you with a team of support workers who ‘click’ with you.
You will be spending a lot of time together in the months and years ahead, so it’s important these are people who over time feel like friends and family.
